EnsureDefaultProjectMembership

EnsureDefaultProjectMembership adds user_id to the customer's default project as a member when the user has no membership there. It creates the default project first when it is missing. It never changes the role of an existing member, and it skips a user whom SCIM blocks from signing in. Idempotent: a repeat call leaves the same state and returns joined=false.
