AlertDestinationService is the (customer_id, workspace_id)-scoped CRUD surface for doctor alert destinations.
Every RPC is permission-ONLY: it declares a permission atom and no legacy
scopes. The interceptor denies a permission-only RPC outright when the
RBAC leg misses, instead of falling through to a coarse scope (see
auth/interceptor.go's permission leg). That is the correct posture for a
surface with no pre-RBAC clients to keep working -- nothing here may be
reached by holding a broad legacy scope alone.
