CreateCatalogServer

CreateCatalogServer adds a server to the MCP catalog, enabled. It never changes an existing one: when a server with this id exists it returns ALREADY_EXISTS and writes nothing. Use UpdateCatalogServer to edit one. It is not idempotent: a client that retries after a lost response and gets ALREADY_EXISTS should read the server with GetCatalogServer.
